Form 1095-C Mailed to Eligible Employees

Harvard is required under the federal Affordable Care Act to provide Form 1095-C to active employees eligible for Harvard group health coverage in 2016. The form, mailed to homes in late February 2017, should be kept with your 2016 tax records (but you do not need to be submit it with your tax return). You can learn more here.
